6-氨基己酸与抑肽酶对心脏瓣膜置换术后血清肌苷清除率的影响 被引量:2

摘要 目的比较心脏瓣膜置换手术时预防性使用6-氨基己酸或抑肽酶对患者术后血清肌苷清除率的影响。方法将109例心脏瓣膜置换手术患者随机分为3组,即6-氨基己酸组(E组,39例)、抑肽酶组(A组,40例)以及空白对照组(C组,30例)。各组患者麻醉、体外循环及心肌保护方法一致。手术时分别给予6-氨基己酸、抑肽酶或生理盐水。测定术前、术后72 h外周血肌酐水平。计算肌酐清除率及其变化率,并以标准体表面积校正。结果:三组患者术前血清肌酐清除率(mL/m in)分别为E组:71.82±14.82;A组:71.90±14.72;C组:72.00±16.10,各组患者之间的差异无统计学意义(P>0.05)。三组患者术后72 h的血清肌酐清除率(mL/m in)分别为(E组:76.57±14.33;A组:78.99±13.60;C组:82.15±18.83),各组患者之间的差异无统计学意义(P>0.05)。E组患者术后72 h血清肌酐清除率差(ΔC rC l72)及其变化率(%ΔC rC l72)中位数分别为:8.07、0.14;A组72 hΔC rC l72和%ΔC rC l72分别为:8.41、0.13;C组患者ΔC rC l72和%ΔC rC l72分别为:5.87、0.11。三组患者ΔC rC l72、%ΔC rC l72之间的差异无统计学意义(P>0.05)。结论6-氨基己酸与抑肽酶对于瓣膜置换心脏手术后手术过程平稳、无手术并发症患者肾功能没有明显的影响。
