摘要
目的 探讨微炎症、营养与颈总动脉内膜中层厚度(CCA-IMT)的关系,寻找预防和早期干预缺血性脑卒中的措施。方法收集250例老年个体,通过B超测定CCA-IMT,并按照CCA-IMT分为5组,即小于0.60mm组、0.60~0.79mm组、0.80~0.99mm组、1.00~1.19mm组和≥1.20mm组;同时测定血清中超敏c反应蛋白(hsCRP)、铁蛋白、白蛋白(ALB)、前白蛋白(pre-ALB)和转铁蛋白(TF)的水平,分析它们与CCA-IMT的相关性。结果反映微炎症状态的血清hsCRP和铁蛋白含量随CCA-IMT值增加而上升,呈正相关(r分别为0.862和0.896,P〈0.01);反映营养状态的血清ALB、pre-ALB和TF含量随CCA-IMT值增加而下降,呈负相关(r分别为-0.633、-0.772和-0.655,P〈0.01)。结论动脉粥样硬化的发生、发展与人体微炎症反应和营养状况密切相关。测定hsCRP和铁蛋白等炎症血清标志物以及ALB、pre-ALB和TF等营养状况指标的含量变化对预测和反映CCA-IMT发展趋势有一定的价值。
Objective To study the relationship of microinflammation, nutrition and common carotid artery intimamedia thickness (CCA-IMT) for the early prevention and interference of ischemic stroke. Methods CCA-IMT was measured by carotid arterial ultrasound in 250 elderly subjects. The levels of serum high-sensitivity C-reactive protein (hsCRP), ferritin, albumin, pre-albumin and transferrin were assayed at the same time. According to the results of CCA-IMT, all the subjects were divided into five groups: 〈0.60 mm group, 0.60-0.79 mm group, 0.80-0.99 mm group, 1.00-1.19 mm group and ≥1.20 mm group. The relationship of hsCRP, ferritin, albumin, pre-albumin, transferrin and CCA-IMT were analyzed. Results As the inflammatory markers, hsCRP and ferritin were significantly correlated to CCA-IMT (P 〈0.01 ). With the increase of CCA-IMT, obvious rising levels of hsCRP and ferritin could be found. As the nutritional markers,albumin, pre-albumin and transferrin were also significantly correlated to CCA-IMT (P 〈0.01 ). The levels of albumin, pre-albumin and transferrin reduced with the increase of CCAIMT Conclusion The genesis and development of atherosclerosis is closely associated with microinflammation and nutrition. It is valuable for us to predict and reflect the developmental trend of CCA-IMT through the changes of these markers.
