皮下CO_2充气对腔镜甲状腺手术循环及血气的影响

摘要 目的观察腔镜下甲状腺手术病人循环和血气监测指标在皮下CO2充气前后的变化。方法ASAⅠ~Ⅱ患者17例,均采用静脉诱导下气管插管、静脉复合麻醉,监测并记录CO2充气前、充气后0.5h、充气后1h及术毕15min时的心率(HR)、血压(BP)、脉搏血氧饱和度(SPO2)、呼气末二氧化碳分压(PetCO2)、血液酸碱度(pH)、动脉血二氧化碳分压(PaCO2)的变化。结果CO2充气后HR、BP、PetCO2、PaCO2明显升高、pH显著降低(P<0.05),而术毕15min以上指标与充气前无显著差别(P>0.05)。结论皮下CO2持续充气可使循环兴奋、使CO2蓄积,但只要在术中严密监测合理处置,就能减少并发症,使异常的循环和血气指标在停止充气后较快地恢复正常,从而提高麻醉的安全性,使患者顺利渡过围术期。
