烧冲伤后不同治疗方案下微循环和总血容量变化比较

摘要 在考虑Fahraeus效应和微循环调节作用的基础上,考察了一套新的计算微循环、大循环和总体血容量变化的方法,并用于烧冲复合伤后早期一次性注入高渗萄聚糖(HSD)与乳酸林格氏液(LR)治疗间的疗效比较;血浆粘度测量值被引入这一计算。结果表明这一新方法通过分别对微循环、大循环血容量的估计,能更确切、更清晰地显示伤后治疗时血容量的变化和分布情况。显示了HSD方案有可能是通过显著增加总血容量,改善大循环、微循环血容量,因而达到维持血压而减轻组织水肿等功效。结果表明,以本文提供的方法为基础发展一种方便实用的临床检测方法是可取的。 Based on considering the Fahraeus effat and the micro-circulatory regulation role, we have used a new calculation method of microcirculation, macrocirculation and total blood volume change(ΔVmic,ΔVmac and ΔVb), to compare fluid therapy effects of hypertonic saline dextran solution (HSD) and lactatic Ringer's solution at the early stage after bum-blast Injury. The measurement of plasma viscosity were used in this calculation. The results show that this new method with the observation of microrocirculation and macrocirculation blood volume change could more exactly and distinctly display change and distribution of blood volume duxing the therapy. It confirm that HSD treatment could increase blood pressure and attenuate tissue edema by significnatly increasing total blood volume, recouping ΔVmac and maintaining VmicThe study suggests that a desirable and practical clinic method can be developed based on the provided method.
