皮层脑电图监测在颅内海绵状血管瘤显微外科中的应用 被引量:1

摘要 目的 探讨术中皮层脑电图(ECoG)监测在颅内海绵状血管瘤继发癫痫患者显微外科手术中的应用价值。方法回顾分析2000~2005年颅内海绵状血管瘤继发癫痫患者15例,术中采用ECoG监测测定致痫灶的准确部位和范围来进行显微外科手术。结果随访发现10例患者停药后1年以上无癫痫发作,4例以术前药量的1/4~1/3量即可控制,1例在药物控制下偶有癫痫发作。结论ECoG监测可发现肿瘤周边的癫痫棘波灶,并指导对癫痫棘波灶进行皮层电灼或多软膜下横切。对控制术后癫痫发作有重要的临床价值。 Objective To investigate the effects of electrocorticography (ECoG) monitoring in microsurgical treatment of intracnmial cavernous bemangiomas (ICH) . Methods Fifteen cases of ICH with secondary epilepsy were investigated from 2000 to 2005. They all had been performed ECoG monitoring during operation for determining the precise position and scope of focal epileptogenicity. Results Followup visit found 10 cases cured without drugs, 4 cases total controlled with 1/4 - 1/3 quantity of premedicant, 1 case had epileptic seizure by chance with drugs. Conclusion ECoG monitoring can provide the direction during the surgical procedure, and controls the postoperative epileptic seizure.
