实时三平面组织多普勒同步成像定量评价正常小儿左室收缩功能 被引量:5

摘要 目的探讨应用实时三平面组织多普勒同步成像(TSI)检测小儿心脏功能的可行性及其临床意义。方法对22例正常小儿的心脏进行实时三平面组织多普勒同步成像,分别利用三平面几何法和双平面Simpson法测量左室射血分数(EF),检测左心室基底和中间段12个节段心肌达到收缩期最大运动速度的时间(Tpv),比较各节段达峰时间顺序和TSI指数。结果同步三平面几何法与双平面Simpson法测得的左室射血分数之间差异无显著性(P>0.05)。正常小儿左室12个节段心肌之间的Tpv之间差异无显著性(P>0.05)。心肌运动的Tpv中间段均小于基底段,但差异无显著性(P>0.05)。TSI指数中呈反方向运动室壁的达峰时间差S-L和S-P均小于60ms。结论实时三平面组织多普勒同步成像用于检测小儿心脏功能可行,可无创、快速地定量评价左室整体和局部节段的收缩功能。 Objective To evaluate the left ventricular systolic function of nomud children by real-time tri-panel tissue synchronization imaging (TSI).Methods 22 healthy volunteers were studied using real-time three panel tissue synchronization imaging at apical four-chamber view, two-chamber view and apical longitudinal view. The Ejection Fraction( EF)was measured by tri-panel geometric rule and bi-pand Simpson's rule. The time to peak velocity (Tpv) of the basal and middle segments of each wall and TSI indexes were acquired.Results There was no statistical significance in EF between the two methods(57.93 ± 6. 60vs58. 45 ± 6.14, P 〉 0. 05) ,and in Tpv between 12 segments of LV( P 〉 0.05). The Tpv of middle segments were shorter than those of basal segments but there was no statistical significance( P 〉 0.05) . The S-L and S-P of TSI index were less than 60 meec. Conclusion Quantitation of the left ventricular systolic function in normal children by real-time Tri-panel tissue synchroniztion imaging is feasible and reliable.
