经直肠CDFI检测前列腺癌局部血流特征与肿瘤血管生长因子表达的相关性研究 被引量:1

摘要 目的探讨经直肠彩色多普勒超声检测前列腺癌局部血流及其与肿瘤血管生成间的关系。方法回顾分析了经直肠前列腺活检证实的前列腺癌患者11例,观察前列腺癌病灶局部的CDFI、PDI血流信号的变化及其分布特征,采用免疫组织化学技术检测病灶局部血管内皮生长因子(VEGF)的表达,并对血流信号和VEGF表达的阳性细胞半定量分级。结果11例患者中经直肠灰阶超声共检出病灶14个,其中单发病灶6个,3例为双侧分布各两个弥漫性病灶,2例未检出病灶。灰阶超声对前列腺癌的诊断符合率为81.8%。CDFI和PDI检查病灶局部血流信号增多,半定量分级多为2级和3级。VEGF表达阳性细胞弥漫分布于癌巢中,在癌巢的中心部位表达强于癌巢边缘,VEGF的表达与CDFI和PDI都较明显的相关。结论彩色多普勒超声对病灶血流信号的分析可在一定程度上反映肿瘤病灶的血管生成特征。 Objective To investigate the correlation between tumor angiogenesis and local blood flow features of prostatic careinoma detected by transrectal ultrasound (TRUS). Methods Eleven patients with prostatic carcinoma confirmed by TRUS guided biopsy were retrospectively reviewed. The changes, distribution and characteristics of local lesional blood flow were observed. The expression of local lesional VEGF was detected by immunohistechemistry. Blood flow signals and positively expressed VEGF cells were semi - quanlificated.Results 14 lesions were found out in 11 patients by TRUS, among them, there were six mono- lesions and six diffused lesions, and two lesions were not detected out at all. The acoordence rate for prostatic carcinoma by grey scale ultrasound was 81.8%. Blood signals in the local lesion increased markedly, and they could be graded as 2 or 3. The expression of VEGF was distributed diffusely in the local carcinoma lesion, which was expressed richer in the center of the lesion than its periphery. The expression of VEGF was significantly correhted with CDFI and PDI signal gradings. Conclusion Analysis of blood flow signals by color Doppler ultrasound could reflect the characteristics of the angiogenesis to a certain degree.
