摘要
目的探讨慢性心力衰竭患者脑钠肽(BNP)升高与右心室功能之间的关系。方法以ELISA法和PHILIPS5500型彩色多普勒超声心动仪,对39例扩张型心肌病(DCM)和40例高血压性心脏病(高心病)患者行血浆BNP和心功能检测。结果在心功能相同状态下,DCM组血浆中BNP明显高于高心病组,两者左心房舒张末期直径(LAEDD)、左心室舒张末期直径(LVEDD)无明显差异,但右心室舒张末期直径(RVEDD)DCM组与高心病组有明显差别(P<0.05)。DCM组中血浆BNP水平与RVEDD呈正相关(r=0.803,P<0.01)。结论在慢性心功能不全的患者,血浆中BNP除与左心功能、LVEDD相关外,与RVEDD也有明显的正相关性。
Objective To discuss the relationship between raised levels of brain natriuretic peptide(BNP) and right ventricular function in the patients with chronic heart failure (CHF). Methods The plasma BNP levels were detected by ELISA and the cardiac function was measured by PHILIPS5500 color Doppler ultrasonic cardiograph in 39 cases of dilated cardiomyopathy ( DCM ) and 40 cases of hypertensive heart disease. Results BNP levels were obviously raised in DCM group compared with hypertensive heart disease group at the same heart function,but there was no significant difference in end-diastolic diameter of left atrium ( LAEDD ) and left ventricle ( LVEDD ) , however,end diastolic diameter of right ventricle ( RVEDD ) in DCM group was significantly different ( P 〈 0.05 ). There was positive correlation between the levels of serum BNP and the end diastolic diameter of right ventricle in the patients of chronic heart dysfunction ( r = 0. 803, P 〈 0.01 ). Conclusion The serum BNP in chronic heart failure patients is correlated with left cardiac function, LVEDD and RVEDD as well.
