摘要
目的探讨CD8+T细胞分泌的Th1/Th2类细胞因子在非小细胞肺癌中的作用。方法将具有完整分期(Ⅰ、Ⅱ、Ⅲ、Ⅳ期)的非小细胞肺癌患者与正常对照组随机各抽取35例,对其CD8+T细胞及其分泌的Th1类细胞因子Interferon-γ(IFN-γ)、Interleukin-2(IL-2)和Th2类细胞因子IL-4的变化进行流式细胞术检查并进行Spearman分析。结果正常对照组、Ⅰ、Ⅱ、Ⅲ、Ⅳ期肺癌中CD8+T细胞表达IFN-γ占CD8+T细胞百分比分别为(61.71±11.41)%、(59.83±11.32)%、(46.28±9.54)%、(35.16±7.51)%和(28.57±5.82)%;IL-2百分比分别为(63.22±12.01)%、(65.28±12.52)%、(54.27±9.78)%、(46.81±7.86)%和(35.24±6.84)%;IL-4百分比分别为(30.59±4.84)%、(31.52±4.97)%、(42.28±6.38)%、(53.37±8.61)%和(67.58±10.88)%;与Ⅰ、Ⅱ期肺癌相比,Ⅲ、Ⅳ期肺癌CD8+T细胞表达Th1类细胞因子IFN-γ、IL-2占CD8+T细胞的百分比下降,表达Th2类细胞因子IL-4占CD8+T细胞的百分比升高,有统计学意义(P值均<0.01)。经Spearman相关分析发现肺癌的分期与CD8+T细胞表达Th1类细胞因子IL-2、IFN-γ占CD8+T细胞的百分比呈负相关,秩相关系数分别为rS=-0.5124;rS=-0.4862;rS=-0.4327。而与CD8+T细胞表达Th2类细胞因子IL-4占CD8+T细胞的百分比呈正相关,秩相关系数分别为rS=0.5056。结论晚期肺癌患者Th1/Th2类细胞免疫功能失平衡,Th1类细胞免疫功能下降。
[Objective] To investigate the change of Th1/Th2 type cytokines of CD8^+T cell in different phrase of non-small cell lung cancer(NSCLC). [Methods] The all target were randomized to acquire and classed into normal, Ⅰ, Ⅱ, Ⅲand Ⅳ groups. Then the Th1/Th2 type cytokines of CD8^+T cell of each group were measured by flow cytometry. At the same time, the spearman correlation was carried out between the 4 groups and the Th1/Th2 type cytokines of CD8^+T cell. [Results] The percentage of CD8^+T cell expressing interferon-γ, IFN-γ/in CD8^+T cell were (61.71± 11.41)%, (59.83±11.32)%, (46.28±9.54)%, (35.167±.51)%, (28.57±5.82)%, respectively; the percentage of CD8^+T cell expressing interleukin-2, IL-2 in CD8^+T cell were (63.22±12.01)%, (65.28±12.52)%, (54.27±9.78)%, (46.81±7.86)%, (35.24±6.84)%, respectively; the percentage of CD8^+T cell expressing IL-4 in CD8^+T cell were (30.59±4.84)%, (31.52±4.97)%, (42.28±6.38)%, (53.37±8.61)%, (67.58±10.88)% in normal group, Ⅰ, Ⅱ ,Ⅲ and Ⅳstage. The percentage of IFN-γ, IL-2 of Th1/Th2 type cytokines of CD8^+T cell in Ⅰ, Ⅱ stage were higher than that in Ⅲ, Ⅳ stage (P 〈0.01) while the percentage of IL-4 were significantly increased (P 〈0.01). The percentage of CD8^+T cell, IFN-γ, IL-2 of Th1/Th2 type cytokines of CD8^+T cell negative correlated with the degrees of NSCLC (rs=-0.5124, rs=- 0.4862, rs=-0.4327, respectively) while the IL-4 of Th1/Th2 type cytokines of CD8^+T cell positive correlated with the degrees of NSCLC (rs=0.5056). [Conclusion] There were lower IFN-γ/, IL-2 of Th1/Th2 type cytokines of CD8^+T cell of peripheral vein blood in Ⅲ, Ⅳ phrase with lung cancer, in which were significantly correlated to the degree of lung cancer. The cell immune was defected in advanced lung cancer.
