质子磁共振频谱分析(^1HMRS)鉴别颅内常见肿瘤 被引量:1

摘要 目的:通过质子磁共振频谱(1HMRS)方法分析脑内常见肿瘤与脑正常组织以及脑肿瘤间各指标的差别,探讨1HMRS鉴别脑内肿瘤的优势。方法:筛选临床病例完整、临床诊断明确的颅内肿瘤患者52例,正常对照组选择体检健康者17名。通过1HMRS分析不同肿瘤的总肌酸(PCr/Cr)、乳酸(Lac)、胆碱(Cho)、N-已酰天门冬氨酸(NAA)的差别,以及各项指标在肿瘤与正常组织之间的差别。结果:正常组织与脑膜瘤、脑胶质瘤和转移瘤之间的NAA/Cr、Cho/Cr、Cr/PCr、Lac/Cr的T检验分别为,NAA/Cho:T脑膜瘤/正常组=12.43,T胶质瘤/正常组=8.10,T转移瘤/正常组=8.17;NAA/Cr:T脑膜瘤/正常组=3.10,T胶质瘤/正常组=2.06,T转移瘤/正常组=8.53;Cho/Cr:T脑膜瘤/正常组=5.20,T胶质瘤/正常组=3.47,T转移瘤/正常组=4.57(P<0.05)。表明这几种常规肿瘤与正常组织中所观察指标比较差异具有显著性。脑膜瘤和脑胶质瘤的1HMRS分析中TNAA/Cho=0.004,脑胶质瘤和转移瘤的TLac/Cr=0.012,脑膜瘤中心和周边组织中TCho/Cr=0.045,胶质瘤瘤周区域与转移瘤瘤周区域TCho/Cr=0.025,(P<0.05)。其他的各项指标并不存在统计学差异。结论:结合磁共振成像(MRI)的MRS对于脑内恶性肿瘤鉴别具有一定的意义,对于区别不同类型的肿瘤有一定的参考价值。
