复杂颅内动脉瘤的个体化血管内介入治疗(附63例报告)

Individualized Endovascular Treatment of Complicated Intracranial Aneurysms :report of 63 cases

目的探讨不同栓塞技术在复杂颅内动脉瘤血管内介入治疗中的适应证、操作规程和临床效果。方法采用常规可脱弹簧圈技术、球囊再塑形技术、支架载瘤动脉成形术及载瘤动脉闭塞术治疗复杂颅内动脉瘤63例(66个)。结果完全或近全栓塞率为69.7%,部分栓塞率21.2%,致残率6.1%,病死率3.0%;27例患者获随访,平均随访时间8.5个月,均无颅内出血或再出血。结论对复杂颅内动脉瘤患者进行个体化血管内治疗疗效确切,值得借鉴。

[Objective] To investigate the indications, operative regulations and clinical effect of different embolic techniques for endovascular treatment of complicated intracranial aneurysms. [Methods] 66 complicated intracranial aneurysms in 63 cases were treated ＂with routine detachable coil technique, balloon remodeling technique, stent endovasculoplasty technique and parent artery occlusive technique. [Results] Complete or nearcomplete occlusion rate was 69. 7%, partial occlusion rate was 21. 2%, [ersistent morbidity was 6. 1%, mortality was 3.0%. 27 patients were followed up for 8. 5 months on an average, none of them suffered from intracranial bleeding or rebleeding. [Conclusions] The effects of individualized endovascular treatment for complicated intracranial aneurysms is reliable and it is deserved to use clinically.
