肌内粘液瘤的临床MRI诊断——附1例报告并文献复习

MRI diagnosis of intramuscular myxoma: one case report and review of literatures

目的探讨肌内粘液瘤的临床病理特点及影像诊断,着重分析MRI诊断价值。方法回顾性分析我院收治1例肌内粘液瘤患者的临床病理及影像学资料。结果患者男,43岁,临床表现为左髋部外侧一8cm×6cm包块,缓慢增大,症状轻微。MRI表现为左髋部臀中肌内占位,大小为8.0cm×7.5cm×4.5cm,边界清楚,T1WI呈均匀低信号、T2WI显著高信号,Gd-DTPA增强后不均匀强化,出现“周围脂肪带”征和周围肌肉内水肿。外科手术切除后无复发、无转移,病理诊断为肌内粘液瘤。结论肌内粘液瘤的临床特点不明显,诊断困难,但MRI具有明显特征和诊断价值。

Ojective To study the pathologic characteristics and image diagnosis of intramuscular myxoma（IM）. Methods The clinical data of one patient with IM in our hospital was analyzed retrospectively. Results The patient was a 43-year-old man with a mass in deeply soft tissue. It was characterized clinically by slow growth and minimal symptoms. The lesion was homogeneously low in signal intensity on T1-weighted MR sequences, and a high signal intensity on T2-weighted MR sequences. The degree of enhancement was most frequently intense and heterogeneous after gadolinium administration, and there were perilesional at rind and edema signal in surrounding muscle on MR images sometimes. The patient achieved the complete recovery by surgical excision. The followingup showed no recurrence and metastasis. Conclusion The diagnosis of IM is difficult. MRI is of great value with obvious imaging features.
