腹腔镜胆囊切除术严重并发症26例报道

Severe complications after laparoscopic cholecystectomy:report of 26 cases

目的探讨腹腔镜胆囊切除术(LC)严重并发症的原因及其对策。方法回顾性分析该科3680例LC各种严重并发症的临床资料。结果胆管损伤6例,胆瘘11例,肠穿孔3例,肝外胆管结石漏诊5例,戳孔肿瘤种植1例。除1例恢复较差外,全部治愈。结论重视胆囊切除指征,注意胆囊三角的解剖关系,是防治各种并发症的关键。腹腔镜技术诊断和治疗LC术后严重并发症有重要价值。

[Objective] To study the causes and countermeasure of severe complications after laparoscopic cholecystectomy （LC）. [Methods] The clinic data of patients of severe complications in 3 680 cases of laparoscopic cholecystectomy in our department were analyzed. [Results] In 26 cases, bile duet is injured in 6 cases; bile leakage in 11 cases; gastrointestinal is injured in 3 cases; stones removed incompletely in 5 cases; port-site＇s cancer implantation in 1 case, [Conclusions] It is important to think highly of the cholecystectomy indication, paying attention to anatomy of gallbladere triangle to reduce severe complication. The important value of laparoscopy in diagnosis and treatment of severe complications after laparoscopic cholecystectomy.
