无创通气在慢性阻塞性肺疾病并Ⅱ型呼吸衰竭中的应用

Application of Noninvasive Bi-level Positive Pressure Ventilation in Patients with COPD and Type Ⅱ Respiratory Failure

目的探讨无创通气在慢性阻塞性肺疾病并Ⅱ型呼吸衰竭中的临床应用。方法对45例慢性阻塞性肺疾病并Ⅱ型呼吸衰竭者进行抗感染、平喘、祛痰等常规治疗的同时,使用无创BiPAP呼吸机治疗,观察治疗前及治疗后1h、4h、24h的血气分析变化和临床结果。结果38例患者使用BiPAP呼吸机治疗后PaCO2明显下降,PaO2、PH明显上升,以治疗1h后的参数变化最明显。结论BiPAP呼吸机适用于慢性阻塞性肺疾病伴Ⅱ型呼吸衰竭患者,可迅速提高患者的氧分压,降低二氧化碳分压及呼吸频率,纠正呼吸衰竭。

Objective Investigation of the effect of noninvasive BiPAP ventilation in patients with CODP and degree Ⅱ type respiratory failure. Methodes 45 cases of degree Ⅱ respiratory failure used the noninvasive BiPAP ventilation during antibiotics, antispasmotic and antitussive treatment. The arterial blood gas contents were examined before treatment and after 1 hour, 4 hours, 24 hours of therapy, Results 38 cases＇s PaO2 and PH using noninvasive BiPAP ventilation could be significantly increased and the PaCO2 was significantly reduced. Conclusion Noninvasive BiPAP ventilation has clinical efficiency on degree Ⅱ respiratory failiure.
