HLA-B27阳性强直性脊柱炎患者免疫功能和血液流变学的检测

The Test of Immune Function and Hemorheologic Changes in HLA-B27 Positive Patients with Ankylosing Spondylitis

目的了解HLA—B27阳性强直性脊柱炎(AS)患者免疫功能和血液流变学的变化。方法采用双抗体夹心ELISA 法检测血清白细胞介素6受体(IL-6)。直接免疫荧光法测定外周血T淋巴细胞亚群(CD3+,CD4+,CD8+),用速率散射比浊法检测免疫球蛋白、C反应蛋白(CRP),同时对患者进行血液流变学检测。结果患者IL-6、CRP和免疫球蛋白明显高于正常组(P<0．05),T淋巴细胞亚群CD3+、CD8+与正常组比较明显降低;CD4+与正常组比较明显升高(P<0．05)。血液流变学检测示患者红细胞压积、纤维蛋白原、全血粘度、血浆粘度和红细胞电泳指数均增加。结论 HLA-B27阳性AS患者存在免疫功能亢进与紊乱,血液流变学有明显的改变。HLA-B27抗原在AS发病过程中起了重要作用,对机体的免疫功能产生一定影响。

Objective To study immune function and hemorheologic changes of HLA-B27 positive patients with ankylosing spondylitis （AS）.Metbods The levels of interleukin-6 （IL-6） were measured by double-mAb sandwich ELISA,T-lymphocyte subsets （CD3^＋,CD4^＋ and CD8^＋） were also evaluated by direct immunofluorescence. IgG,lgA, IgM and C-reactive protein （CRP） in serum were detemined by rate nephelometry.Patient＇s hemorheologic changes were tested.Results Serum IL-6,IgG,IgA,IgM and CRP levels in patients were significantly higher than those in normal subjects （P〈0.05）; As compared with controls,CD3^＋ and CD8^＋ were significantly lower in patients; CD4^＋ were significantly higher in patients （P〈0.05）.Hematocrit, capiUary fibrinogen,whole blood apparent viscosity, plasma viscosity,electrophoresis index of RBC were significantly higher in cases of patients than those in normal subjects. Conclusions Immune functional imbalance and up-regulation exist in HLA-B27 positive AS patients.There are distinct changes of hemorheology.HLA-B27 antigen may play an important role in progressive stage of AS patients and produce certain influence on organism of immune function.
