全腹腔镜下带血管蒂回肠襻移植阴道成形术的临床应用

Study of operative procedures of laparoscopic vaginoplasty using avascularized pedicledi leal flap

目的探讨全腹腔镜下带血管蒂回肠移植阴道成形术的方法及其临床应用效果。方法回顾性分析2004年1月至2005年10月13例全腹腔镜下带血管蒂回肠移植阴道成形术患者的临床资料。结果13例手术均成功,术后近期发生肠梗阻1例,经再次手术治愈;阴道口狭窄2例,分别经扩张、阴道口成形术后痊愈。10例随访3￣6个月,7例无异味,3例轻度异味;6例有性生活,5例满意,1例有少量出血。结论全腹腔镜下带血管蒂回肠移植阴道成形术创伤小,较符合生理状态,疗效良好,是目前较理想的阴道成形方法。

Objective To explore the procedures and the clinical significance of total laparoscopic vaginoplasty using a vascularized pedicled ileal flap. Methods A retrospective analysis was made on 13cases of total laparoscopic vaginoplasty by using a vascularized pedicled ileal flap. Results Operations were successfully completed in all the 13cases. Short-term complications in the immediate postoperative period included lease of intestinal obstruction,which was cured by a secondary surgery, and 2 cases of stricture of vaginal orifice,which were cured by a vaginal dilatation and an orifice reconstruction respectively. 10 cases were followed up for 3-6 months. No abnormal smelling vaginal discharge were found in 7cases and mild abnormal smelling in 3cases. Among 6 patients who had sexual life,5 were satisfied with their sexual experience, 1 had small amounts of hemorrhage. Conclusions Total laparoscopic vaginoplasty using a vascularized pedicled ileal flap is safe and effective. It may meet most expectations of the patients, It is an ideal approach of minimal invasive vaginoplasty.
