摘要
本文对20例精索静脉曲张致不育症患者和10例已生育的精索静脉曲张患者在行左精索静脉高位结扎术中,同时取左精索静脉及周围静脉血作血气分析比较,并与患者术前精液分析对照。发现两组的术中精索静脉血与周围静脉血PO2(氧分压)、SaO2(血氧饱和度)、PCO2(二氧化碳分压)、pH及其它指标没有显著性差异(P>0.05)。患者精索静脉血中有PO2下降、SaO2下降、PCO2上升等变化,与精索静脉曲张致男性不育精液检查所表现出的不同程度的精子数减少、活动率降低、畸形精子比例增加等变化没有必然联系(P>0.05)。初步分析其临床意义认为:精索静脉曲张中的睾丸血流动力学改变以及血气平衡失调致不育的假说,即精索静脉血淤滞和睾丸组织缺氧在精索静脉曲张所致不育中的作用,在本临床研究中没有得到验证。
PO 2、SaO 2、PCO 2、pH in internal spermatic veins and peripheral veins were measured in 30 men with varicocele, including 20 infertile men and 10 fertile, during high ligation of internal spermatic veins. Comparing with their preoperative semen analysis, the results show: 1. there are no significant differences of PO 2、SaO 2、PCO 2、pH between internal spermatic veins and peripheral veins ( P >0.05) in infertile group and fertile group. 2. there are no significant differences between the changes of PO 2、SaO 2、PCO 2 in internal spermatic veins and the changes of semen analysis in infertile men with varicocele, such as teratozoospermia, asthenozoospermia, oligozoospermia ( P >0.05). The unbalance of blood gas in internal spermatic veins in men with varicocele, one of the theories about infertile mechanism caused by varicocele, cannot be proved in our clinicae study.
