Clinical Application of Double-helical CT Angiography and MR Angiography with Multi-model 3D Reconstruction in Diagnosis of Arteriovenous Malformation of Brain

摘要 目的:评价双层螺旋CT血管造影(DCTA)与MR血管造影(MRA)多模式三维重建诊断脑动静脉畸形(AVM)的临床价值。材料与方法:临床疑AVM患者75例行DCTA和MRA检查,源像输入HP4.1工作站,采用多平面重建(MPR)、最大密度投影(MIP)、表面遮盖显示(SSD)和容积再现(VR)等技术重建。分析各种显示效果及诊断效能,与DSA结果对照。结果:MPR、MIP、SSD能准确显示AVM的大小、部位和范围。仿真血管内窥镜(VM)对AVM的空间解剖结构等显示具有良好效果。DCTA和MRA诊断AVM的敏感性、特异性和准确性各为93.3%、100%、96%及82.2%、93.3%、86.7%。结论:DCTA及MRA能在短时间内完成脑血管容积数据采集,二者的多模式三维重建对诊断AVM具有重要价值。 Purpose: To evaluate the clinical value of double- helical CT angiography (DCTA) and MR angiography (MRA) with multi - model 3D reconstruction .in diagnosis of arteriovenous malformation (AVM) of brain. Materials and Me/hods: DCTA and MRA were performed on 75 patients who suspected worrying AVM. THe source images were transferred to HP 4.1 workstation, and reconstructed with muhiplanar reconstruction (MPR), maximum intensity projection (MIP), shaded surface display (SSD) and volume rendering (VR). Analyzed the effect of display and the diagnostic effcacy. Results: The magnitude, location and morphology of AVM could be accurately showed with MPR, MIP and SSD. VM could provide information of good anatomic structure space of AVM. The sensitivity, specificity and accurately of DCTA and MRAfor examined AVM were 93.3%, 100%, 96% and 8212%, 93.3%, 86.7%, respectively. Conclusion: DCTA and MRA could demonstrate a longer segment of intmeranial vessels with a thinner Slice and a shorter scan time. The multi- model 3D reconstruction of DCTA and MRA was valuable in diagnosing AVM of brain.
