慢性心力衰竭与微量蛋白尿附30例临床分析

摘要 目的 探讨慢性充血性心力衰竭伴微量白蛋白尿与左室结构、功能及其预后的关系。方法 55例冠心合并慢性充血性心衰病人,按其尿白蛋白排泄量将病人分为两组:正常白蛋白尿(NAU)组25例(24h尿白蛋白〈30mg)、微量白蛋白尿(MAU)组30例(24h尿白蛋白30—300mg),分析比较两组心血管疾病危险因素、心电图和超声心动图变化。结果 MAU组较NAU组心电图异常的发生率明显升高,EF值显著下降,以LVEF〈45%计算,MAU组发生率明显升高。结论慢性充血性心衰病人,MAU的出现标志着左室收缩功能的显著减退,心脑血管事件发生率显著增加。 Objective Explore the relationship between configuration and function of left ventricle and microalbuminuria in congestive heart failure (CHF). Methods According to urinary albumin excretion rate (UAER), 55 patients with heart failure and hypertension were divided into two groups. 25 patients were in normal albuminuria (NA) group (UAER〈30mg/24hour) and 30 patients were in microalbuminuria (MAU) group (UAER 30-300mg/24hour). The risk factor in diseases of heart and brain, EKG, Color Doppler echocardiography between the two groups were analyzed. Result Compared MA group with NA group, the incidence of abnormal EKG in MA group increased significantly (P〈0.05), and the EF decreased, meanwhile, the incidence of LEVF which is below the 45% in MA group increased. Conclusion The function of left ventricle in congestive heart failure patients with MA was more decreased than those with NA in the same disease The incidence with brain and heart were increased.
