刺五加浸液离子导入治疗糖尿病周围神经病变临床观察 被引量:4

Clinical Observation on Diabetic Peripheral Neuropathy Treated by the Imported Extract Ion of Ciwujia

摘要 目的:采用刺五加浸液离子导入治疗糖尿病周围神经病变(DPN),并进行电生理观察。方法:339例DPN病人,分为实验组、静注组、电疗组和对照组,均常规糖尿病治疗。实验组(87例)采用刺五加浸液经皮离子导入治疗;静注组(82例)应用刺五加注射液静脉输注;电疗组(91例)病变局部皮肤应用直流电疗法;对照组(79例)应用常规抗凝、扩血管及营养神经治疗。对四组病人进行临床症状观察,同时检测正中神经、尺神经和腓总神经的感觉神经传导速度(SCV)及运动末端潜伏期(MLP),并进行治疗前后自身疗效对照。结果:实验组各临床症状好转率均在90%以上,其中肢体疼痛、麻木、发凉好转率达100%;静注组好转率平均为59%;电疗组好转率平均为60%;对照组好转率平均为24%。组间比较有显著差异(P<0.01,P<0.05)。实验组正中神经、尺神经和腓总神经的SCV及MLP较静注组、电疗组和对照组均明显缩短,自身对照及组间比较均有明显差异(P<0.01)。四组治疗后血糖(空腹及餐后2 h)、糖化血红蛋白(HbAlc)、肝肾功能、血脂及血流变学检测均有不同程度的好转,但组间比较无差别。结论:刺五加浸液可明显提高正中神经、尺神经和腓总神经的SCV及MLP,说明可在一定程度上促进受损神经组织的修复和再生,改善神经髓鞘功能,降低DPN的致残率。 Objective:To treat diabetic peripheral neuropathy (DPN) by importing extract ion of Ciwujia and to have electrophysiological examination. Methods:339 cases of patients with DPN were divided into the experimental group, the intravernous drip group. the electrotherapy group and the controlled group. All the groups were given regular diabetic treatment. The experimental group (87 cases) were treated with the imported extract ion of Ciwujia, while the intravenous drip group (82 cases) with intravenous drip of Ciwujia injection. The electrotherapy group (91 cases) were treated with direct-current electrotherapy on the local diseased skin, while the cortrolled group (79 cases) with anticoagulant therapy, vessel dilatation and nourishing nerve. The symptoms of the patients of four groups were observed and the sensorineural conductive velocity (SCV) and the moverment laten period(MLP) of median nerve, ulnar nerve and common peroneal nerve were examined, and the therapeutic effectiveness was compared before and after the treatment. Results:the improving rates of all the symptoms of the experiment group were above 90%, among them, the improving rates of the pain; numbness and coldness of limbs were above 100%. The average improving rates of the intranenous drip group, the electro-therapy group and the controlled group were respectively 59.2%, 60% and 24%. There was a significant difference among groups (P〈0.01,P〈0.05). SCV and MLP of median nerve, ulnar nerve and common peroneal nerve of the experiment group shortened evidently compared with those of other three groups. There was obvious difference (P〈0.01) between the contrast of the group itself and the comparison among groups. After the treatment of four groups, blood sugar (fasting and 2h after meal), HbAlc;liver and kidney function, blood-fat and blood rheology test had the improvement of different degrees, but there were no difference among groups. Conclusion:Ciwujia extract can evidvertly improve SCV and MLP of median nerve, ulnar nerve and common peroneal nerve, which indicates that it can promote the repainment and regeneration, improve the function of neurilemma and decrease of disability rate of DPN.
