保留血管的阴式筋膜内非脱垂全子宫切除术式探讨

Transvaginal Intrafascial Hysterectomy with Preserving Uterine Arteries

目的探讨保留血管的阴式筋膜内非脱垂全子宫切除术的临床实用价值。方法回顾性分析2004年5月～2005年5月27例子宫肌瘤需切除子宫患者行保留双侧子宫动脉的阴式筋膜内全子宫切除术的临床资料。结果经阴道完成手术27例,手术时间平均58(50～84)min,术中失血平均135(80～250)ml,术后排气时间16(11～30)h,术后病率3.7%(1/27)。术后3个月恢复性生活25例,病人满意度达92%(23/25)。结论子宫肌瘤保留血管的阴式筋膜内全子宫切除术效果可靠、恢复快、并发症少、创伤小,有利于提高患者生活质量,有很好的临床实用和推广价值。

Objective To investigate the clinical value of transvaginal intrafascial hysterectomy （TVIFH） with preserving uterine arteries for uterine myoma. Methods 27 cases with myoma underwent TVIFH of preserving uterine arteries were retrospectively reviewed. Results The average operating time was 58 （50 - 84） min, the mean operating blood loss was 135 （80 - 250） ml, the mean postoperative gastrointestinal function recovery time was 16（ 11-30）h and postoperative morbidity was 3.70% （1/27）. Recovering of sex life 3 months postoperatively were 25 cases and the rate of patients＇ satisfaction up to 92% （23/25）. Conclusion TVIFH of preserving uterine arteries is a procedure with good short-time effect, minimum operation injury, low complications and can improve the quality of life for uterine myoma.
