外路法上睑提肌缩短治疗瘢痕性上睑下垂

Outer pathway shortening of levator palpebrae superioris in treating cicatricial ptosis

目的探讨外伤或义眼台取出术后致上睑提肌瘢痕粘连所致上睑下垂的手术治疗。方法8例(8眼)采用外路法上睑提肌缩短前徙术治疗因上睑提肌瘢痕粘连所致的上睑下垂,术中充分游离上睑提肌腱膜。结果8例(8眼)睑裂高度与对侧眼基本一致,上睑弧度自然,义眼台术后的上睑下垂稍小于对侧眼效果更好。结论外路法上睑提肌缩短前徙可有效治疗上睑提肌瘢痕粘连所致上睑下垂。

Objective To investigate the surgical treatment of ptosis induced by levator palpebrae superior muscle cicatricial adhesion after trauma or intraorbital implantation being taken out of, Methods Outer pathway shortening and anterior transposition of levator palpebrae superior muscle was used in treating ptosis induced by levator palpebrae superior muscle cicatricial adhesion, and the aponeurosis of levator palpebrae superior muscle being freed sufficiently during the surgery. Results The height of palpebral fissure of all patients was the same as that of the contralateral eye, and arc of upper lids were natural. The outcome may be better if the upper lid is slightly lower than the contralateral eye in patients after orbital implantation. Conclusions Outer pathway shortening and anterior transposition of levator palpebrae superioris is usefull to treat ptosis induced by levator palpebrae superior muscle cicatricial adhesion.
