人工胃底折叠重建His角预防贲门癌术后反流82例临床分析

Rebuild the His angle through artificially folding fundus ventriculi to prevent reflux after operation of cardiac carcinoma in the clinical analysis of 82 cases

目的:探讨人工胃底折叠重建His角术式预防贲门癌术后反流的价值。方法:贲门癌手术中利用人工胃底折叠重建His角,达到恢复食管胃连接部的生理功能,从而起到抗食管反流的目的。手术后通过上消化道钡餐及胃镜检查来评定其抗反流效果。结果:全组82例,无手术死亡,无吻合口狭窄及吻合口瘘,上下切缘均无癌残留。其中10例有反流症状,反流率12.2%。对照组53例,有反流者22例,反流率41.5%。经统计学处理两者反流率差别有显著性意义(P<0.0001)。结论:该术式具有较好的抗反流作用,而且具有操作简单、安全等优点。

Objective：To study the value of rebuilding the His angle through artificially folding fundus ventriculi to prevent reflux after operation of cardiac carcinoma. Methods：The His angle was rebuilded through artificially folding fundus ventriculi to prevent reflux during operation of cardiac carcinoma, and attained to recover physiology function of the esophagus-stomach＇s connection in order to reach the purpose of preventing reflux towards esophagus. After operation, assessed the effect of its anti-reflux through barium meal and gastroscope. Results：82 cases had no the surgical operation death, and had no narrow anastomosis junction, and had no anastomosis junction fistula and had no the remnant caranoma the up and down of the junction. Only 10 cases had the reflux condition of illness ,and the reflux rate was 12. 2% ;the control group had 53 cases,there were 22 cases who had the reflux condition of illness and the reflux rate was 41.5%. There was sinificant difference between the two groups（ P 〈 0. 000 1 ）. Concluslon：That operation style has a good anti-reflux function, and has advantages of the operation simple, safety etc.
