关于改革和完善医院补偿机制的思路

文章分析了目前医院经济补偿成分及医疗收费标准中知识技术劳务成分低的问题;提出医院经济补偿机制应与社会大众经济承受能力相适应,必须有利于区域卫生规划,并与国家对补贴政策和医疗收费价格调整同步进行。为此,医院财政补贴的政策应该是重组医疗卫生资源,由铺摊子向质量效益内涵建设转变;引进竞争机制,根据完成工作任务状况给予财政补贴;国家财政补贴向基层卫生机构倾斜;在调整医疗收费价格标准时,应加大知识技术劳务费比例,按医院等级定医疗收费价格标准。

The current composition of hospital fees and the low percentage of intellectual and tech- nical elements in the composition are analyzed.The medical payment demanded by the hospital should be what the public can bear within the rein of the state financil assistance policy and within the control of the medical payment regulations.The state policy of financial assistance to hospitals should be one focusing on medical and health resources,transferring from one favoring quantitative expansion of the public health service to one orientated toward the qualitative improvement.The mechanism of competi- tion is introduced and the amount of financial assistance proportional to the work done.The grassroots health units are better taken care of in such assistance.The intellectual and technical elements should have a higher proportion in the composition of medical payment which varies with the grade of a hospi- tal.
