摘要
目的:探讨平阳霉素在腮腺局部注射对面神经的影响。方法:将实验兔随机分成A、B两组,每组12只。A组左侧为实验侧,B组左侧为阳性对照侧,A、B两组右侧为阴性对照侧。常规麻醉,暴露面神经的上下颊支,并在其周围注射相应的药物,A组左侧注射平阳霉素,B组左侧注射消痔灵,两组右侧注射生理盐水。然后分别在术后第1、3、6周进行面神经功能观察、神经传导速度检测、组织学观察。结果:(1)A组两侧术后各周期内未出现面神经功能障碍。B组左侧则在术后第3周出现明显的面瘫,第6周无明显恢复。(2)A组左侧在术后各周期内面神经传导速度无明显下降,P>0.05。B组左侧则在第3、6周面神经传导速度下降明显,P<0.05。(3)在光镜和透射电镜下观察,术后A组左侧神经组织学形态结构各实验周期内基本正常。而B组左侧在术后第3周出现明显的神经变性,6周时略有恢复。结论:平阳霉素在家兔面神经周围注射后不会造成面神经变性,这为在腮腺区应用平阳霉素治疗血管瘤提供了理论上依据。
Objective:To discuss whether PTM can cause irreversible injuries to facial nerve through injecting PYM into the facial nerve area of parotid.Methods:24 experimental rabbits were divided randomly into 2 groups(A and B) ,12 per group. The left side of group A were the experimental side, The left side of group B is the positive control side, The right side of both groups serves as the blank control group.After routine anesthesia, expose the superior and inferior buccal branches of the facial nerve and inject corresponding drugs, the left side of group A is given PYM and the left side of group B is given Xiaozhiling, the right side of both groups are given NS. And then Observations on the functional activity of facial nerve, detection the nerve conduction velocity and observation on histology was studied at 1,3and 6 week after operation.Result: (1)both side of the facial nerve within group A shows no dysfunction or facial paralysis 1,3,6 weeks postoperative. Left side of group B shows obviously dysfunction developed 3 and 6 weeks postoperative that manifested in facial paralysis. The dysfunction deesn' t alleviated 6 weeks later. (2) The nerve conduction velocity of Left side of group A declined lightly, P 〈 0.05.That of left side of group B declined signifisenfly, P 〉 0.05. (3) Observation by light and transmission electron microscope, The nervous tissue from the left side of group A shows nearly normal at any experimental stage. Group B shows obviously nervous degeneration 3rd week postoperative, though the degeneration was alleviated slightly at the 6th week. Conclusion: PYM cannot cause dysfunction of the facial nerve after injection near the facial nerve of rabbit, This can be used as the theoretic basis to inject PYM at the parotid area for treatment on hemangioma.
