有限内固定结合外固定架治疗桡骨远端不稳定骨折

Limited internal fixation combined with external fixation stent for the treatment of radial distal unstable fracture

目的探讨涉及关节面的桡骨远端关节内粉碎性骨折的治疗方法。方法自2003年3月-2005年11月对16例桡骨远端关节内粉碎性骨折采用外固定架结合有限内固定,必要时行有限切开复位内固定治疗。结果疗效按Dienst功能评估标准:优11例,良3例,可2例,优良率87.5%。结论涉及关节面的桡骨远端粉碎性骨折在手法复位不良佳时,应积极手术;采用在手法整复基本恢复桡骨远端长度及各种移位的基础上,外固定架结合有限内固定,必要时有限切开直视下撬拨复位内固定。

Objective To investigate the therapeutic method of radial distal intra-articular comminuted fracture involved articular surface. Methods From March 2003 to November 2005, 16 cases with radial distal intra-articular comminuted fracture underwent external fixation stent combined with limited internal fixation. Results According to Dienst functional score standard, the excellent result was in 11 cases,good 3 cases and fair 2 cases.The excellent and good rate were 87.5%. Conclusion When the manual reduction is not satisfactory for involving articular surface of radial distal comminuted fracture,the operative treatment should be done aggressively. The operative methods are using manual reduction for restoring length of radial distal end and correcting displacement,and at the same time,the external fixation stent combined with limited internal fixation should be performed .If manual reduction is fail, the limited reduction under direct vision and internal fixation are necessary.
