血管内皮生长因子在早期异位妊娠及宫内胚胎停育的表达

The expression of serum vascular endothelial growth factor in ectopic and arrested intrauterine pregnancies.

[目的]通过检测血清血管内皮生长因子(VEGF)、绒毛膜促性腺激素(HCG)在宫内早孕(IUP)、宫内胚胎停育(aIUP)及异位妊娠(EP)中的不同浓度,探讨VEGF在停经早期鉴别EP及aIUP的意义。[方法]选择孕40d左右妇女,采用酶联免疫聚合反应(ELISA)检测20例正常IUP妇女(正常对照组)、15例aIUP(自然流产组)及20例EP妇女血清VEGF和HCG的浓度,并进行对比分析。[结果]血清VEGF浓度EP组为(295.6±52.1)pg/m l,明显高于IUP组的(40.4±21.5)pg/m l和aIUP组的(110.3±34.5)pg/m l,差异均有显著性(P<0.01,P<0.05);HCG在EP组为(2092.8±1634.1)m Iu/m l、aIUP组为(2352.4±1712.8)m Iu/m l,都明显低于IUP组的(14780.6±8777.3)m Iu/m l,差异均有显著性(P<0.01),而EP组与aIUP相比差异无显著性(P>0.05)。[结论]VEGF在EP的高表达将有助于EP的早期诊断,VEGF可用于鉴别早期EP与aIUP,与HCG相比具有优越性。

[ Objective] To evaluate serum vascular endothelial growth factor （VEGF） and HCG levels in women with normal intrauterine pregnancy （IUP）, arrested IUP （alUP） and eetopie pregnaneies （EP） at early gestation. [Methods] A prospective ease - eontrol study was eondueted comparing serum VEGF and HCG eoneentrations among 50 women with early pregnaneies who subsequently had 20 normal IUP, 15 arrested IUP and 20 EP . [ Results ] There was a signifieant difference in VEGF eoneentrations among women with EP, arrested IUP and normal IUP. VEGF showed statistieally signifieant differenees between EP and the other 2 groups （ P 〈 0.05 ） ; HCG showed statistieally signifieant differenees between normal IUP and the other 2 groups （ P 〈 0. 01 ）, but not between EP and arrested IUP （ P 〉 0. 05）. [ Conclusion ] VEGF ean diseriminate EP from arrested IUP at early pregnaneies while HCG eannot. VEGF is a potential marker for early diagnosis of EP.
