椎-基底动脉供血不足与经颅多普勒及颈椎横突孔的相关性研究

目的探讨椎-基底动脉供血不足(VBAI)与经颅多普勒(TCD)及颈椎横突孔之间的相关性。方法对150例VBAI受试者同时进行TCD检查和颈椎CT及X线平片检查。结果TCD常规检测阳性率为70·7%,加做颈转试验使阳性率提高25·3%;横突孔扫描检出相对性狭窄25例,继发性狭窄38例。结论(1)对血流速度在正常范围者应常规加做转颈试验,以提高其阳性率;(2)CT、磁共振血管成像(MRA)和X线平片反映了横突孔与椎动脉颅外段的关系,若与TCD检测的颅内段进行综合判断,互为补充,可从影像学的角度为VBAI的诊断和手术治疗提供客观的依据。

Objective To determine the correlation between VBAI and TCD,eervieal transverse foramen. Methods TCD,CT and X-ray examination of cervical vertebra were simultaneously done in 150 cases of VBAI. Results The positive rate of routine TCD exam is 70.7 % ,while adding cervical rotating test,it will increase 25.3 %. Twenty five eases with relative stenosis, thirty eight eases with secondary stenosis were found by CT scan of corviacl transverse foramen. Conclusions （ 1 ） For the cases with normal blood velocity, cervical rotating test should also be done in order to increase the positive rate. （2） CT, MRA and X-ray exam reflect their relation with extracranial vertebral artery,and if with combining analysis of its intracranial part by TCD it will provide objective data for the diagnosis of VBAI and surgical treatment.
