牙冠延长术在老年患者残根修复中的应用

The application of crown lengthening in the restoration of residual root in old patients

目的:比较牙冠延长术和牙龈切除术在老年患者牙断端折断至龈下的残根修复中的应用疗效。方法:选取我科就诊的老年临床病例,经检查患牙牙龈部分或全部覆盖牙根断面,随机分为两组,实验组30例采用牙冠延长术,对照组30例采用牙龈切除术;术后6周行桩冠修复。结果:术后6周复查,牙冠延长术的手术疗效与牙龈切除术相比,两者之间差异有显著性;1年后复诊,牙龈切除术组桩冠修复后并发症的发病率高于牙冠延长术组。结论:牙冠延长术和牙龈切除术应用于老年患者残根修复中,均能明显改善龈袋深度,但前者优于后者。

Objective： To analyze the clinical effects of crown lengthening and gingivectomy of residual root on deep-seated subgingival in old patients. Methods： 60 old patients with 60 remnant root were selected and randomly divided into two groups： crown lengthening surgical group （30cases） and gingivectomy group （ 30 cases）. The roots were restored with post crowns routincly 6 weeks after operation. Results： At 6 weeks follow up, the effective rate between crown lengthening group （6 weeks, 100%） and gin givectomy ＂group （6 weeks, 80%） was statistically different. After one year, the complication of crown lengthening group （12 mon 11.2%） was lower than that of gingivectomy group （12 mon, 42.86% ） and was statistically different. Conclusion： Both gingivectomy and surgical crown lengthening could shorien the depths of the gingival pocket in the old patients. The clinical effect of the former is better than that of the latter for the present.
