放射免疫法检测笑气无痛分娩对垂体泌乳素影响

摘要 目的人血清垂体泌乳素为垂体前叶分泌的蛋白激素,在雌激素、孕激素、皮质类固醇和胰岛素作用基础上,对泌乳的开始和维持起着重要作用,妊娠后血清中垂体泌乳素逐渐增加,分娩前达高峰,哺乳期进一步增加.采取放射免疫检测笑气(50%)无痛分娩对产妇泌乳素的影响.方法选择妊娠38~42周,无严重产科并发症,无乳腺发育缺陷及分泌病的初产妇60例,随机分为两组,Ⅰ组为50%笑气无痛分娩组30例;Ⅱ组为未用任何麻醉药经阴道自然分娩组30例.用放射免疫法测定产后5min,24,48h血清泌乳素值,两组产妇产后24h内开始泌乳发生率(40%)48h内开始泌乳及72h乳汁分泌不足发生率相比无差异.结果与结论50%笑气无痛分娩不影响产后母乳喂养成功率. Objective The human serous pituitary prolactin(PRL) is a protein hormone secreted by anterior lobes of pituitum. Based on estrogen, pregnanediol, cortical steroid and insulin, PRL takes important role in beginning and maintaining of milk secretion. After pregnance, pituitary PRL is gradually elevated and reached high peak before the delivery, and further increased during lactation period. The effect of 50% Xiaoqi on painless childbirth by detecting pituitary prolactin(PRL) of puerperal with Radioimmunoassay. Methods 60 cases 38- 42 weeks primipara who were without severe obstetrical complication, mammary aplasia and secretion disease were chosen. They were randomly divided into 2 groups(30 cases for each group). In group Ⅰ, painless group, 50% Xiaoqi was used. In group Ⅱ , any anesthetics was not used and childbirth was given through vagina. The value of serous PRL was detected by radioimmunoassay at 5 minutes, 24 hours and 48 hours of postpartum. There is no difference between 2 groups on 24 hours and 48 hour beginning milk secretion of postpartum and 72 hours milk secretion insufficiency. Results and Conclusion There is no effect on breast-feed success ratio of postpartum by using 50 % Xiaoqi to make painless childbirth.
