p53蛋白在宫颈上皮内瘤变和宫颈癌中的表达

Expression of p53 protein in cervical intraepitheliai neoplasia and carvical carcinoma and its relationship with lymph node metastasis

目的探讨p53蛋白在子宫颈上皮内瘤变和宫颈癌中的表达及淋巴转移的关系。方法采用免疫组化SP法分别对34例子宫颈上皮内瘤变、40例宫颈癌、10例正常宫颈组织进行p53蛋白表达状况的检测。结果p53蛋白在正常宫颈组织、子宫颈上皮内瘤变、宫颈癌中的阳性表达率差异有显著性(P＜0．05)。在子宫颈上皮内瘤变Ⅰ中,p53蛋白低表达,与正常宫颈组织的阳性表达率差异无显著性(P=0．481＞0．05),与宫颈癌的阳性表达率差异有显著性(P＜0．05)；在子宫颈上皮内瘤变Ⅱ中,p53蛋白的表达率明显增高,与正常宫颈组织的阳性表达率差异有显著性(P=0．002＜0．01),与宫颈癌的阳性表达率差异无显著性(P＞0．05)。p53蛋白的表达在淋巴转移组与非淋巴转移组差异无显著性(P=0．565＞0．05),既p53蛋白的表达与宫颈癌的淋巴转移无关。结论p53的过表达与宫颈癌的发生发展有关,可作为宫颈癌早期诊断和子宫颈上皮内瘤变治疗指导及预后判断的一项有效指标。

Objective To study expression of p53 protein in cervical intraepitheliai neoplasia （CIN） and carvical carcinoma and its relationship with lymph node metastasis. Methods p53 protein expression was detected by using immunohistoehemieal staining in 34 patients with CIN ,40 patients with cervical carcinoma and 10 normal eontrols. Results The expression of p53 in cervical carcinoma was significantly higher than that in CIN and normal cervical tissues respectively （beth P 〈 0.05 ）. The expression of p53 in CIN Ⅰ was not significantly higher than that in normal cervical tissues （ P 〉 0.05 ）, and there was a significant difference as compared with that in cervical carcinoma tissues （ P 〈 0.05）. In CIN Ⅱ, the expression of p53 sharply increased and there was significant difference as compared with that in normal cervical tissues （ P 〈 0.05 ）, but there was not significant difference as compared with that in cervical carcinoma tissues （ P 〉 0.05 ）. In expression of p53, there was no significant difference between lymph node metastasis group and non lymph node metastasis group, that is, the expression rate of p53 was not statistically related to lymph node metastasis （P 〉 0.05） in cervical carcinoma. Condusion The over-expression of p53 is associated with occurrence and development of cervical carcinoma,it may be an important indiee for early diagnosis, treatment and evaluating prognosis of CIN.
