摘要
目的:了解颅内压增高后肺表面活性物质(PS)改变,探讨PS变化在呼吸功能不全中的作用。方法:采用硬膜外囊注入生理盐水升高犬颅内压至8kPa(1kPa=7.5mmHg),测量血气和支气管肺灌洗液(BALF)物理特性。结果:动脉血氧分压及BALF最大表面张力(rmax)显著降低,最小表面张力(rmin)显著增加,滞环面积(HA)缩小,白蛋白增加,肺含水量增加。结论:颅内压增高后肺表面活性物质减少,PS测定有助于颅内压增高后呼吸功能损害的早期判断。
Aim: To probe into the changes of pulmonary surfactant (PS) after intracranial hypertension and effect of PS on pulmonary dysfunction. Methods: The intracranial pressure in dogs was raised abruptly to 8 kPa by injecting saline into extradural balloon. Blood gas and physics property of bronchoalveolar lavage fluid (BALF) were measured. Results: Arterial O 2 tension and maximal surface tension decreased significantly. Minimal surface tension increased and hysteresis area (HA) dropped. There was a marked increase of albumin in BALF and lung water. It was proposed that PS was decreased after intracranial hypertension. Conclusion: The determination of PS may serve as an early diagnosis of pulmonary dysfunction after intracranial hypertension.
