机械通气治疗重症急性左心衰16例疗效观察

The Observation on the Mechanical Ventilation Treatment of Patients with Acute Severe Left Ventricular Failure

目的评价机械通气治疗重症急性左心衰竭的疗效。方法对16例急性重症左心衰竭患者在吸氧、镇静、强心、利尿、扩血管等常规治疗无效后紧急气管插管机械通气,观察上机后pH、PaO2、PaCO2、SaO2、R、HR、MAP各项指标,并与上机前比较。结果13例重症急性左心衰竭患者的pH、PaO2、PaCO2、SaO2、R、HR、MAP各项指标比通气前有明显改善,差异有显著性意义(P<0.05或<0.01)。结论机械通气是抢救重症急性左心衰竭的一种有效措施。

Objective Investigate the clinical effectiveness of mechanical ventilation in the treatment of acute severe left cardiac failure. Methods 16 patients with acute severe left ventricular failure were treated with mechanical ventilation at correct time besides other conventional medical therapy. Results 13 patients （81%） were improved remarkably in symptoms and analysis of blood gas showed that PH, PaO2, PaCO2 and SaO2 changed significantly （ P 〈 0. 05 ）. Respiratory rates were much better than before. Conclusion Prompt mechanical ventilation is an effective method in successfully rescuing patients with acute severe left heart failure.
