首发精神分裂症患者IL-1β的血清蛋白水平及PBMC mRNA表达 被引量:1

摘要 目的从基因转录和蛋白水平,观察精神分裂症中IL1β的动态变化情况,了解其免疫状态,并探讨IL1β与精神病理的关系。方法采用病例对照研究方法,精神分裂症组为根据DSMⅣ诊断为精神分裂症的本地汉族患者83例,单一应用利培酮治疗,在治疗前后评定PANSS量表;对照组为健康志愿者65例。应用ELISA和RTPCR方法,同时检测血清IL1β蛋白水平和PBMC中IL1βmRNA表达。结果(1)精神分裂症组治疗前、后与对照组血清IL1β水平相比,均明显增高,差异有显著性(P<0.05)。精神分裂症组治疗后血清IL1β水平与治疗前相比明显下降,差异有显著性(P<0.05)。(2)IL1β治疗前后的差值与PANSS量表降分率呈显著正相关(r=0.388,P<0.05)。(3)精神分裂症组治疗前、后PBMC中IL1βmRNA表达与对照组相比明显增高,差异有显著性(P<0.05)。精神分裂症组治疗后PBMC中IL1βmRNA表达与治疗前相比明显下降,差异有显著性(P<0.05)。结论(1)精神分裂症患者存在免疫激活,IL1β水平的变化可作为病情缓解的一个灵敏指标。(2)精神分裂症患者PBMC呈激活状态,是IL1β的主要来源之一,探讨影响PBMC细胞因子mRNA表达的因素,有助于反映对机体的免疫状况的影响。 Objective To measure in parallel the serum levels and the Peripheral Blood Mononuclear Cell (PBMC) mRNA expression of IL-1β both before and after exposure to risperidone during a 4-week treatment protocol for schizophrenia, and to determine the immune state of schizophrenia and whether the levels of these factors were correlated with clinical variables. Methods The subiects were 83 patients who met the DSM-IV diagnostic criteria for schizophrenia and 65 healthy control subjects. All patients were treated with single risperidone. The psychopathological status of each schizophrenia patient was assessed using the PANSS before and 4-week after the start of neuroleptic administration. For all subjects, serum IL-1β levels were estimated using commercial ELISA kits according to the manufacturer's instructions. Applying reverse transcription-polymerase chain reaction(RT-PCR) technique to semiquantitatively analyze IL-1β mRNA expression in the PBMC. Results (1) Serum IL-1β levels in the schizophrenia group at both before and after 4-week treatment with risperidone were significantly higher than those in the control subjects (P〈0.05). Serum IL-1β levels of the schizophrenia patients decreased significantly after 4-week treatment with risperidone(P〈0.05). (2) There was significant correlation between the difference of serum IL-1β levels before and after treatment and the rate of the decreased PANSS(r=0. 368, P〈0. 05). (3) IL-1β mRNA expression in the PBMC from the schizophrenia group at both before and after 4-week treatment with risperidone were significantly higher than those from the control subjects(P〈0.05). IL-1β mRNA expression in the PBMC from the schizophrenia patients decreased significantly after 4-week treatment with risperidone(P〈0.05). Conclusion (1) Schizophrenia patients are in a state of immune activation. The difference of serum IL-1β level between before and after treatment with neuroleptic may be as a sensitive parameter of illness recovery. (2) PBMC of schizophrenia patient is activated and one of maior sources of IL-1β. To explore the related factors of mRNA expression of IL-1β in the PBMC may be useful to understanding the immune mechanism of illness.
