摘要
目的探讨瘦素及血管内皮生长因子(vascular endothelial growth factor,VEGF)在胃癌组织中的表达及临床意义。方法采用免疫组化SP法检测50例胃癌及30例切缘区正常胃组织中瘦素、VEGF的表达。结果①胃癌及切缘区正常胃组织中均有瘦素及VEGF的表达,胃癌组织中瘦素及VEGF表达的阳性率高于切缘区正常胃组织(72.00%,60.00%vs 46.67%,36.67%,χ2=5.13,4.09,P<0.05);②有淋巴结转移、浆膜浸润及分期较晚(Ⅲ/Ⅳ期)的胃癌组瘦素表达阳性率高于无淋巴结转移、无浆膜浸润及分期较早(Ⅰ/Ⅱ期)的胃癌组(82.35%,80.49%,91.67%vs 50.00%,33.33%,53.85%,χ2=4.16,5.97,8.85,P<0.05);③有淋巴结转移、浆膜浸润及分期较晚(Ⅲ/Ⅳ期)的胃癌组VEGF表达阳性率高于无淋巴结转移、无浆膜浸润及分期较早(Ⅰ/Ⅱ期)的胃癌组(73.53%,68.29%,79.17%vs 31.25%,22.22%,42.31%,χ2=8.10,4.75,7.06,P<0.05);④胃癌组织中瘦素与VEGF表达呈正相关(rs=0.404,P<0.01)。结论胃癌组织中瘦素和VEGF表达增多,二者的表达水平与胃癌的淋巴结转移、浆膜浸润及TNM分期密切相关;瘦素可能因协同VEGF或通过上调VEGF的表达而在胃癌的浸润、转移中发挥了一定的作用。
Objective To investigate the expression of leptin and vascular endothelial growth factor(VEGF) in gastric cancer(GC) tissues and their roles in the biologic behavior. Methods The expression of leptin, VEGF were measured by immunohistochemical staining (SP) in 50 GC eases of them and 30 eases of them with margin normal tissues. Results OThe expression of leptin and VEGF in GC tissues was significantly higher than that in margin normal tissues(72.00 % vs 46.67 %, 60.00 % vs 36.67 %, χ^2 = 5.13,4.09, respectively, P〈0.05). ② The positive rates of leptin in GC with serosal invasion and lymph node metastasis and in GC advanced stage(stage Ⅲ + Ⅳ ) were significantly higher than those without serosal invasion and without lymph node metastasis or in early stage(stage Ⅰ + Ⅱ )(82.35%, 80.49%, 91.67% vs 50.00%, 33.33%, 53.85%, respectively, P〈0.05). ③The positive rates of VEGF in GC with serosal invasion and lymph node metastasis and in GC advanced stage (stage Ⅲ + Ⅳ) were significantly higher than those without serosal invasion and without lymph node metastasis and in early stage(stage Ⅰ + Ⅱ)(73.53%, 68.29%, 79.17% vs 31.25%, 22.22%, 42.31%, respectively, P〈0.05). ④Correlation analysis showed that there was a significantly positive correlation between the expression of leptin and VEGF( r =0. 404, P〈0.01). Conclusion The expression of leptin and VEGF in GC tissues is higher than that in normal tissues. Leptin might be responsible for the enhanced activity in tumor invasion and metastasis via up-regulating the expression of VEGF or cooperating with VEGF.
