老年胃癌腹腔内温热灌注化疗

Intraoperative and Postoperative Intraperitoneal Hyperthermic Chemotherapy for Gastric Cancer in Eldly

目的:探讨老年胃癌根治术中及术后腹腔内温热灌注化疗的临床疗效及对肝功及血液系统的影响。方法:在均行化疗德基础上,将术中、术后腹腔温热灌注化疗的60例老年胃癌患者(治疗组)与未行腹腔内温热灌注化疗的71例患者(对照组)的腹腔游离癌细胞检出率(ICC)、肝功能、血细胞分析进行对比。结果:治疗组和对照组的游离癌细胞检出率分别是3.8%和32.6%,术后肝功能、血液细胞分析两组相比无明显差异(P>0.05),治疗组术后1、2、3年内生存率分别为100%、85%、64.2%;对照组则为95%、51.3%、32.4%,两组2、3年生存率相比有显著意义(P<0.01)。结论:术中及术后腹腔内温热灌注化疗在老年胃癌患者可明显杀灭腹腔游离癌细胞,降低复发率,提高生存率。

Objective To investigate the clinical effect of intraoperative and postoperative peritoneal hyperthermic chemotherapy in advanced gastric cancer in eldly patients. Methods Sixty cases in treatment group with advanced gastric cancer in eldly patients received intraoperatitve and postoperative intraperitoneal hyperthermic chemotherapy and other 71 cases in control group received only saline water lavage after radical resection. Isolated cancer cells （ICC） in peritoneal fluid, hepatic funcion,bloed cell analysis were compared between two groups. Results The positive rate of ICC was 3.8% in treatment group and 32.8% in the control group （ P 〈 0.01 ）, hepatic function and blood cell analysis had no significant differences in two groups and 1,2,3 -year survival rates were 100% ,85% ,64.2%, and 95% ,51.3% ,32.4% in treatment and control groups, respectively. There were significant differences in the 2,3 - year survival rates between two groups （P 〈 0.01 ）. Conclusions Intraoperative and postoperative intraperitoneal hyperthermic chemotherapy is safe in eldly patients with gastric cancer,it can kill isolated intaperitoneal cancer cells, reduce recurrence and improve the prognosis.
