摘要 目的:研究充血性心力衰竭(CHF)患儿的脑钠肽(BNP)的变化及意义。方法:CHF患儿入院当天、治疗15天、出院后3个月测定血BNP水平。分析BNP水平变化与再住院事件的关系。结果:BNP浓度与NYHA心功能分级呈正相关,3个月内共发生临床终点18例,其入院时BNP水平(969.3±248.2)pg/ml,明显高于未发生临床终点者(549.5±202.1)pg/ml。发生临床终点患儿出院时BNP水平(942.3±421.0)pg/ml,较入院时升高;未发生临床终点患儿出院时BNP水平(389.6±245.8)pg/ml,较入院时降低。结论:BNP的检测有利于CHF患儿病情评估,其疗效评价具有临床价值。 Objective: To investigate the value of plasma brain natriuretic peptide (BNP) in children with CHE Methods: The BNP levels of children with CHF were detected the day admitted in this hospital, 15 days after treatment and 3 months after discharge to analyze the relationship between BNP level change and readmission. Results: The serum BNP concentration in children with CHF was positively correlated with NYHA class. In three months, 18 endpoints occurred cardiac death or readmission. BNP levels in poor and good outcomes were (969.3±248.2) pg/ml and (549.5±202. 1) pg/ml, respectively (P 〈0.01). BNP levels inpatients with endpoints increased (942. 3±421.0) pg/ml when discharge than that in admission; that of inpatients without endpoints decreased (389. 6±245. 8) pg/ml when discharge than that in admission (P 〈0. 01 ) . Conclusion: BNP is an useful marker for illness evaluation and therapeutic effect of CHF.
