断流术后门静脉血栓形成与血小板膜蛋白CD62P的关系

Relationship between Platelets Membrane Glucoprotein CD62P with Thrombosis of Portal Vein after Devascularization Operation

目的探讨断流术后门静脉血栓形成与血小板膜蛋白CD62P的关系。方法选取肝硬化门脉高压症行断流术的患者30例作为实验组,同期行胆囊切除术的患者15例作为对照组。于术前1天及术后第1天测定静脉血中血小板CD62P水平,并行血小板计数;实验组行彩色多普勒超声检查,了解门静脉系统血栓形成情况。结果实验组术后门静脉系统血栓形成6例。术前实验组CD62P高于对照组(P<0.01);术后实验组CD62P明显高于术前(P<0.01)。血栓组与无血栓组术前CD62P的差异无显著性意义(P>0.05);术后血栓组CD62P升高更明显(P<0.01)。结论门脉高压症断流术后CD62P明显升高,且与门静脉系统血栓形成高度相关,可作为评判门静脉系统血栓形成高危的敏感指标。

Objective To investigate the relationship between the levels of platelets membrane glucoprotein CD62P with thrombosis of portal vein after devascularization operation. Methods Thirty patients with portal hypertension and liver cirrhosis who had a devascularization operation were selected and served as experimental group, while 15 patients with cholecystectomy at the same period as control group. The levels of platelets membrane glucoprotein CD62P were detected one day before and after operation in each group. Platelets were counted. Color Doppler ultrasound examination was performed in experiment group after operation to know thrombosis of portal vein. Results There were 6 patients with thrombosis of portal vein after operation. The CD62P levels in experimental group were significantly higher than in control group before operation （P〈0.01）, and after operation it was higher than before in experimental group （P〈0.01）. There was no significant difference in CD62P level between thrombosis group and no thrombosis group before operation （P〈0. 05）, but after operation, CD62P was increased more obviously in thrombosis group than in no thrombosis group （P〈0. 01）. Conclusion The level of CD62P was increased significantly after devascularization operation in patients with portal hypertension and closely correlated with thrombosis of portal vein, so it could be used as a sensitive mark to evaluate high risk patients of thrombosis of portal vein.
