子宫内膜非典型性增生的手术疗效探讨

Evaluation on surgical curative effect of atypical hyperplasia of endometrium

目的探讨宫腔镜子宫内膜切除术(TCRE)对子宫内膜非典型增生疾病的治疗效果。方法分析了30例TCRE治疗子宫内膜非典型增生的疗效。结果术后44%无月经,24%经量减少,16%正常月经,16%复发,治疗有效率84%(21/25)。子宫内膜逆转率85.71%(18/21),正常率76.19%(16/21),癌变率为4%(1/25)。结论TCRE治疗子宫内膜非典型增生效果较满意,疗效中轻度好于中、重度增生,二者差异存在显著性。对于年轻、有生育要求或身体状况无法耐受子宫切除术者提倡首选TCRE术。

[Objective] To investigate the curative effect of atypical hyperplasia of endometrium by transcervical resection of endometrium （TCRE）. [Methods] Clinical data of 30 patients with atypical hyperplasia of endometrium were analyzed after TCRE. [Results] 44% had amenorrhea, 24% had hypomenorrhea, 16% had normal menstrual flow, 16% were failed after surgery. 84% were effective. 85.71% of Endometrium had reversed, 76.19% had normal Endometrium. 4% run to endometrial adenocarcinoma. [ Conclusion] The data suggest that the treatment of atypical hyperplasia of endometrium by TCRE have satisfying effect. The postoperative curative effect of light atypical hyperplasia of endometrium is significantly higher than middle and serious one. It should be the first choice for the patients of young or infertility or to be not able to bear hysterectomy.
