摘要
目的结合临床病理探讨5-LOX和VEGF在结肠癌中的表达情况,分析5-LOX在结肠癌血管生成方面的作用及其对结肠癌预后判断的临床意义。方法对99例结肠癌患者进行预后随访,用S-P免疫组化法检测结肠癌组织及14例结肠腺瘤和35例正常结肠黏膜组织中的5-LOX和VEGF蛋白的表达情况及计数MVD。结果(1)结肠癌的术后生存时间与结肠癌的Duke分期、有无转移和术后化疗关系密切(P<0.05)。(2)5-LOX和VEGF在结肠癌组织中的阳性表达率及MVD计数显著高于正常结肠黏膜组织(P<0.05);5-LOX阳性表达与结肠癌的Duke分期、浸润深度和有无转移关系密切(P<0.05);VEGF阳性表达与结肠癌的Duke分期和有无转移显著相关(P<0.01)。(3)结肠癌组织中5-LOX阳性表达率和VEGF阳性表达率显著相关;5-LOX、VEGF阳性表达率与结肠癌中MVD计数显著相关(P<0.05)。(4)5-LOX和VEGF表达与结肠癌术后生存时间无关(P>0.05)。(5)在多因素分析中肿瘤转移情况是唯一影响结肠癌预后的独立因素。结论5-LOX和VEGF在结肠癌组织中过度表达,5-LOX与结肠癌的Duke分期、浸润深度和有无转移关系密切,并与结肠癌的血管形成相关,可作为反映结肠癌生物学行为和判断预后的有效指标。
Objective To investigate the expression of 5 - lipoxygenase and VEGF in colonic carcinoma and it' s relationship with tumour pathological characteristice ; To determine whether 5 - LOX expression correlate with tumor angiogenesis and identify it' s clinical value in predicting prognosis of colonic carcinoma. Methods 99 patients with colonic carcinoma were followed up. The expression of 5 - lipoxygenase and VEGF were detected by immunohistochemical staining, as well as MVD counting in 99 colonic carcinoma tissue specimens, 14 adenoma tissue specimens and 35 normal colonic mucosal tissue specimens. Results ( 1 ) The survival time of patients was correlated significantly with Duke' s stage, metastasis of colonic carcinoma and treatment with chemotherapy( P 〈0.05 ). (2)The positive rate of 5 -lipexygenase and VEGF, MVD counting in colonic cancer was significantly higher than those in normal colonic mucosa tissues( P 〈 0..05 ). The expression of 5 - lipexygenase was correlated significantly with the Duke' s stage, depth of invasion and metastasis of colonic carcinoma (P 〈 0.05 ). The positive rate of VEGF was correlated significantly with the Duke' s stage and metastasis of colonic carcinoma( P 〈 0.01 ). (3) The positive rate of 5 - lipexygenase was positively correlated with the expression of VEGF in colonic carcinoma( P 〈0.05 ) ;The positive rate of 5 -lipexygenase and VEGF were positively correlated with MVD counting in colonic carcinoma. (4)No relationship of the expression of 5 -lipoxygenase and VEGF with survival time of patients. (5) Multivariate analysis indicated that metastasis was only independent prognostic factor of colonic carcinoma. Condusion The overexpression of 5 - lipoxygenase and VEGF in patients with colonic carcinoma can enhance the possibility of metastasis, and implicate a poor prognosis ;5 -LOX expression are related to tumor angiogenesis in colonic carcinoma, it may serve as the fairly good prognosis factors to indicate biologic behavior and prognosis in colonic carcinoma.
