碳酸酐酶Ⅸ和血管内皮生长因子在膀胱移行细胞癌中的表达及意义

摘要 目的探讨碳酸酐酶Ⅸ(CAⅨ)和血管内皮生长因子(VEGF)在膀胱移行上皮癌的表达和意义。方法应用免疫组化技术,检测膀胱移行上皮癌和正常膀胱粘膜中CAⅨ和VEGF的表达,结合临床病理资料进行分析。结果56例膀胱移行上皮癌标本中,CAⅨ阳性表达47例(83.9%),VEGF阳性表达49例(87.5%),主要位于膀胱腔面及坏死区周围癌细胞,10例正常膀胱粘膜中未见CAⅨ和VEGF表达。CAⅨ在G3级的阳性表达率高于G1、G2级(P<0.05),在T2-4组高于Tis-1组(P<0.05),VEGF在T2-4组高于Tis-1组(P<0.05)。CAⅨ和VEGF的表达存在正相关性(P<0.05,r=+0.543)。结论CAⅨ和VEGF参与膀胱癌的发展过程,进展期的膀胱癌具有较高的CAⅨ和VEGF表达,二者存在正相关性,缺氧和血管再生是恶性肿瘤的重要标志。 Objectives To investigate the expression and role of carbonic anhydrases Ⅸ(CA Ⅸ) and vascular endothelial growth factors(VEGF) in bladder transitional cell carcinoma. Methods Pathologically diagnosed bladder transitional cell carcinoma and normal bladder mucosa tissue were enrolled in this study. The immunohistochemistry was used to detect the expression of CAⅨ and VEGF. The immunostaining data was compared with clinical pathological outcomes. Results Among the 56 specimen of bladder transitional cell carcinoma, expression of CA Ⅸ was detected in 47 specimen (83.9%) and VEGF in 49 specimen (87.5%). Both of CAⅨ and VEGF were expressed maximally on the luminal surface of tumors and around regions of necrosis in tumors. No positive staining of CAⅨ and VEGF were found in normal bladder tissue. There were significant difference of positive staining rate in CAⅨ between G3 and G1, G2(P〈0. 05), T2-4 and Tis-I (P〈0. 05), in VEGF between T2-4 and Tis-1 (P〈0. 05). There was a association between CA Ⅸ and VEGF(P〈0. 05, r = +0.543).Conclusion CA Ⅸ and VEGF were involved in the genesis and progression of bladder transitional cell carcinoma. The expression of CA Ⅸ and VEGF was closely associated with high grade tumors. The hypoxia and angiogenesis are important markers of carcinoma.
