磁共振尿路成像对上尿路梗阻病变的诊断价值

摘要 目的探讨磁共振尿路成像(MRU)对诊断上尿路梗阻的价值。方法总结本院25例上尿路梗阻患者接受MRU检查的临床资料。结果25例患者经过MRU检查均能显示梗阻的部位,其中肾盂输尿管连接部狭窄4例,MRU均清晰显示尿路形态;输尿管结石15例,13例可见结石影,2例结石被高信号尿液掩盖;医源性输尿管狭窄4例,MRU均可显示梗阻部位及尿路扩张的形态;输尿管息肉2例,MRU显示输尿管梗阻部位可见泡沫样充盈缺损。MRU检查梗阻部位准确率100%。结论MRU用于诊断上尿路梗阻病变,有其优势,也有其缺点,应合理应用。 Objective To evaluate the diagnostic efficacy of magnetic resonance urography(MRU) in patients with upper urinary tract obstruction. Methods 25 patients with upper urinary tract obstruction were subjected to MRU. Results The sites of urinary tract obstruction were all distinctly shown including: Uretero-Pelric junction obstruction(4),ureteral calculi(15), iatrogenic ureteral stricture (13) and ureteral polyp(2).MR urography provided high-resolution images of the kidneys and urinary tract in all patients. The characteristics of renal tuberculosis, urinary tract dilation and the various anatomin anomalies were depicted well. MR urography was successful in localizing the level of obstrution in all of the 35 patients(100%),although MR urography alone could not reliably determine the cause of obstruction in some cases. Conclusion In diagnosing upper urinary tract obstruction diseases, MR urography has its advantages as well as limitations. It should be used appropriately in diagnosing urinary diseases.
