糖肾合剂对早期糖尿病肾病患者尿微量白蛋白排泄率及内皮素-1和一氧化氮的影响 被引量:1

Observation of TangShenHeJi on UAER of patients in early diabetic nephropathy and its influence on plasma endothelin levels and nitrogen monoxide

摘要 目的观察糖肾合剂对早期糖尿病肾病(DN)患者尿微量白蛋白排泄率(UAER)及血浆一氧化氮(NO)和血浆内皮素-1(ET-1)的影响。方法76例早期DN患者随机分为对照组和治疗组各38例,2组继续原有降糖治疗,血压高者予硝苯地平缓释片降压。在此基础上,治疗组应用糖肾合剂治疗,对照组则应用洛丁新治疗。30日为1个疗程,连用3个疗程,观察2组UAER及NO和ET-1变化。结果2组治疗后UAER、ET-1均明显下降(P<0.01,P<0.05);治疗后2组比较无显著性差异(P>0.05);治疗组治疗后NO水平明显下降,与治疗前比较有显著性差异(P<0.05);对照组治疗后NO水平虽有下降,但与治疗前比较无显著性差异(P>0.05);治疗后2组比较有显著性差异(P<0.05)。结论糖肾合剂可明显降低早期DN患者UAER、ET-1和NO水平,其降低早期DN患者UAER的机理与其降低ET-1、NO水平有关。 Objective To observe the influence of TangShenHeJi on urinary micro albumen excretion rate (UAER), plasma endothelin levels (E-T-1) and nitrogen monoxide (NO) on patients in early diabetic nephropathy. Methods 76 patients were selected in this study and randomly divided into 2 groups, the TangShenHeJi group and the control group. Each group has 38 cases. The blood glucose and blood pressure of all the cases in the two groups were treated to a satisfactory level. Then the cases in TangSbenHeJi group were treated with TangShenHeJi and the cases in control group were treated with Lotensin (BenazePril Hydrnchloride Tablets). Results After the trealment, the urinary micro albumen excretion rate (UAER) and ET-1 of all the cases in the two groups decreased markedly ( P〈0.05), and there was no significant difference between the two groups( P〉0.05). The NO in cases of TangSbenHeJi group decreased markedly after the treatment (P〈0.05). The NO in cases of control group decreased little, and there was no significant difference compared with the NO in the same group before the treatment( P〉0.05).There was a significant difference between the two groups after the treatment in NO ( P〈0.05). Conduction The results proved that TangShenHeJi can greafly decrease the UAER of the patients in early diabetic nephropathy and its effect on UAER and ET-1 has no significant difference compared with the control group. Besides, TangSbenHeJi can also decrease the NO of the patients and there is a significant difference between the control group. The conceivable mechanism of TangShenHeJi in decreasing the UAER of the patients in early diabetic nephropathy has something to do with its effect of decreasing the ET-1 and NO.
