复杂性肛瘘术前造影的诊治价值

The Diagnostic Value of Preoperative Fistulography of Complicated Anal Fistula

为了探讨复杂性肛瘘的诊断方法，以提高手术一次性治愈率，降低复发率，将126例复杂性肛瘘随机分为2组，治疗组63例采用76％复方泛影葡胺和2％亚甲蓝混合液作术前瘘管造影检查。对照组采用常规探针及术中探查等。治疗组术前了解瘘管形态、走向、深浅及内口数目和位置，一次治愈率95．2％，高于对照组（60.3％），复发率低于对照组（4．8％VS 39．7％）。结果表明，术前瘘管造影检查在复杂性肛瘘诊治方面有明显优点。

The objective of the study was to probe a useful diagnostic method for complicated anal fistula, with the attempt to help enhance the primary cure rate and reduce recurrence. All the 126 cases with complicated anal fistula were randomly and evenly divided into two groups. Sixty three cases in the study group underwent preoperative fistulography with the mixture of meglucamine diatrizoate （76%） and methylen （2%）,while the other 63 cases in the controlled group underwent routine probe examination ＆ midoperatire exploration. As results,the primary cure rate and recurrence rate in the study group were more satisfactory（95.2% vs 60.3% and 4.8% vs 39.7%） than that in the controlled group,due to the reason that fistulography ensured a better preoperative understanding of feature, branching,depth location arid number of internal openings. It is revealed that preoperative fistulography is of notable strong points in the diagnosis of complicated anal fistula.
