肝癌的螺旋CT多期增强表现与血清中VEGF表达的关系

摘要 目的:探讨肝癌的螺旋CT(spiralCT,SCT)多期强化特征与血清中血管内皮生长因子(vas-cularendothelialgrowth factor,VEGF)含量的关系。方法:对20例经病理学证实的肝细胞癌患者的SCT三期强化特征,如肿瘤大小、包膜、侵袭危险性和强化类型等进行分析;用ELISA法检测各患者血清中VEGF的含量,并同20例正常人进行对照。结果:肝癌组血清VEGF明显高于正常对照组(P<0.01),血清VEGF的总阳性率为75%。血清VEGF的表达与肝癌强化特征、包膜完整性均有较显著性差异(P<0.05),与肝癌的大小无显著性差异(P>0.05);肝癌增强动脉期的明显均匀强化组的血清VEGF阳性率明显高于中等环形强化组和轻度或无强化组(P<0.05)。结论:肝癌的SCT多期强化特征与血清中VEGF含量有一定的相关性,联合观测能提高对肝癌侵袭和转移评估的能力。 Objective: To explore the correlation between features of enhancements by multiphase contrast-enhanced spiral CT and the serum VEGF level in hepatocellular carcinoma. Methods: In 20 cases with HCC were proved by pathology, analyse the three-phase enhanced SCT features including size.capsule.infiltration and enhancement patterns of each patient and compare the VEGF levels in serum samples including 20 cases with HCC and 20 healthy cases were detected quantitatively with ELISA respectively. Results: The serum VEGF level of Hepatocellular Carcinoma was significantly higher than that of healthy cases (P〈0.01). The total positivity rate of VEGF was 75% (15/20). The expression of the serum VEGF in HCC was significantly elevated as compared to the enhancement features and capsule (P〈0.05) ,While no correlation with the size of HCC (P〉0.05); The serum VEGF level in HCC was obviously higher in the group of high symmetrical enhancement than that of medium ring-shape enhancement and slight or no enhancement at the arterial phase (P〈0.05). Conclusion: There was certain relativity between the multipbase enhanced SCT features and the serum VEGF level in HCC. Both of them could be elevated at the same time in the patients with HCC will appear to reflect its potential activity of vascular invasion and metastasis.
