摘要
目的用放射性核素99mTcO4-SPECT动态显像观察结膜松弛症泪液排泄系统,帮助判断临床治疗方案的选择,评价治疗效果。方法24例40眼结膜松弛症患者(分组每级各10眼),用99mTcO4-10μCi/0.01 ml对泪液排泄系统进行动态显像,16眼手术切除松弛结膜后再次SPECT显像并观察泪溢情况改善。结果随着结膜松弛程度的加重即分级的增加,泪液排泄时间延长即T1/2值(median transit time)增大,Ⅲ、Ⅳ级与正常对照组T1/2值差异有显著性(P=0.000 1),Ⅲ、Ⅳ级与Ⅰ、Ⅱ级T1/2值之间有显著性差异。手术前与手术后T1/2值差异有显著性(P=0.000 1)。结论结膜松弛症患者泪液排泄延缓,并且随着病情的严重程度即分级的增加,患者泪液排泄时间延长即T1/2值增大,可以通过放射性核素99mTcO4-最低安全浓度10μCi/0.01 ml SPECT进行临床辅助诊断。结膜松弛症患者手术后99mTc-SPECT示泪液排泄恢复正常,可作为Ⅲ、Ⅳ级结膜松弛症患者手术疗效的评估。
Objective To observe the lacrimal drainage system of conjunctivochaliasis with the lowest safety dose of ^99mTc by using SPECT, to direct the clinical treatment option of conjunctivochaliasis and evaluate the treatment effect. Methods The lacrimal drainage conditions of 24 patients （40 eyes） with conjunctivochaliasis （10 eyes in each group according to clinical diagnostic degree） were observed by using SPECT. 16 eyes with surgical operation were followed by post operation SPECT observation and clinical observation. Results With the relaxing degree of conjuctiva, the median transit time （T1/2） of the lacrimal drainage was gradually prolonged as measured by 10 μCi/0.01 ml dose of 99mTc with- SPECT. There＇ s significant difference （P = 0. 000 1 ） of T1/2 of degree Ⅲ group and of degree Ⅳ group than volunteer. Significant difference of T1/2 between degree Ⅲ/Ⅳ groups and degree Ⅰ/Ⅱgroups existed. So did significant difference of T1/2（P = 0. 0001） between pre-operation and post operation. Conclusion The lacrimal drainage is prolonged in the conjunctivochaliasis patients. With the increase of serious dergee of coniunctivochaliasis, the lacrimal drainage time prolongs, which can be diagnosed ttirough the lowest safety dose of ^99mTc with SPECT and to help the clinical diagnosis. ^99mTc-SPECT shows that lacrimal drainage might recovere to normal after surgical operation. It can help to evaluate the operation efficacy for patients with coniunctivochaliasis of Ⅲ,Ⅳdegree.
