大面积头皮缺损并颅骨外露的治疗

Treatment of Large Area Epicranium Defect and Cranial Bones Reveal

目的探讨大面积头皮缺损并颅骨外露的治疗方法。方法32例大面积头皮缺损并颅骨外露患者，早期仅作简单清创、换药，1周左右颅骨外板钻孔，1～3个月肉芽覆盖整个颅骨植皮，或头皮扩张以自体头皮覆盖创面。结果32例患者均治愈，住院时间2～4个月，平均2．8个月，5例以自体头皮覆盖创面，保持了头发的完整。结论大面积头皮缺损并颅骨外露的治疗应早期进行创面处理，并尽可能以自体头皮覆盖。

Objective To explore treatment of large area epicranium defect and cranial bones reveal. Methods Thirty-two cases with large area epicranium defect and cranial bones reveal were chosed. Debridement and chang dressings were done in the early, outer table of skull was perforated at a week, during the first month to the third month after injury, granulation was used to cover the revealed cranial bones or autoallergic epicranium was used to cover the skull by dilator. Results The 32 patients were all healed and the average time of hospitalization was 2 - 8 months, five of them were treated with their own epicraniums and the integrity of the hair was restored. Conclusion To cover the revealed skull is the first aim of the treatment of large area epicranium defect and cranial bones reveal and try to use the autoallergic epicranium possibly.
