彩色多普勒超声对肝癌肝动脉化疗栓塞的疗效评价

Evaluation of efficacy of transcatheter arterial chemoembolization in treatment of hepatocellular carcinoma by color Doppler flow imaging

目的探讨彩色多普勒超声(CDFI)在评价肝癌肝动脉化疗栓塞(TACE)疗效中的应用价值。方法对68例原发性肝癌TACE术前、后行CDFI检查,观察肿瘤大小、血供变化情况。结果TACE术后大多数肿瘤均有不同程度缩小;肿瘤内部及周边血供明显减少(P<0.05),以肿瘤内血流减少明显,其血供减少程度与肿瘤缩小程度相一致。结论CDFI能客观判断肿瘤大小及血流灌注状态,是目前评价肝癌TACE效果较理想的影像学检查方法,具有预后判断的作用和指导重复治疗的价值。

Objective To disscuss the application value of color Doppler flow imaging（CDFI） in evaluating efficacy of transcatheter arterial chemoembolization （TACE） in treatment of hepatocellular carcinomon. Methods CDFI was performed in 68 patients with hepatocellular carcinoma before and after TACE. The size and blood supply of the tumor were assessed. Results After TACE, a majority of the tumor volume obviously reduced, the blood flow within and around the tumor obviously decreased （ P 〈 0.05 ） and the changes of the intratumor blood supply were more significant than those of blood supply around the tumor. A positive correlation was observed between the decrease in blood flow and the reduction of tumor volume. Conclusions CDFI can assess objectively the size and the blood flow in liver tumor and is a good imaging technique in evaluating efficacy of TACE in treatment of hepatocellular carcinoma at present. CDFI appears to be a useful method for evaluating prognosis and guiding further therapy of liver cancer patients after TACE.
