CK19、PCNA和P53在甲状腺病变中的表达

摘要 目的:了解CK 19、PCNA、P 53在甲状腺病变中的表达及其诊断意义。方法:采用免疫组化SP法检测60例甲状腺病变(甲状腺乳头状癌36例,结节性甲状腺肿乳头状增生14例,甲状腺瘤10例)中CK 19、PCNA、P 53蛋白的表达。结果:CK 19在36例甲状腺乳头状癌中呈阳性表达,多为强阳性;14例结节性甲状腺肿乳头状增生中3例阳性,呈小灶性阳性,阳性表达者为伴有真乳头的乳头状增生;10例甲状腺瘤中4例阳性,呈小灶性阳性和点状阳性表达,阳性表达者为增生活跃的小滤泡腺瘤和不典型腺瘤。36例甲状腺乳头状癌中30例PCNA阳性,多为中等或强阳性表达,14例结节性甲状腺肿中4例PCNA阳性,10例甲状腺腺瘤中3例PCNA阳性,多为弱阳性,PCNA在甲状腺乳头状癌阳性表达比结节性甲状腺肿和甲状腺腺瘤高,差异有统计学意义(P<0.01)。36例甲状腺乳头状癌中15例P 53阳性,多为弱阳性表达,在结节性甲状腺肿和甲状腺瘤中P 53均呈阴性表达,经两两比较的z检验,三种病变中的P 53表达差异无显著性(P>0.05)。结论:局灶性CK 19阳性可见于良性病变,弥漫性阳性是甲状腺乳头状癌的特征,有助于隐匿性乳头状癌与良性结节的鉴别诊断和甲状腺组织内是否存在微小转移灶的判断。PCNA对甲状腺病变的鉴别诊断起参考作用,P 53在甲状腺病变的鉴别诊断中意义不大。
