瑞芬太尼复合丙泊酚麻醉应用于介入手术 被引量:1

摘要 目的 观察瑞芬太尼复合丙泊酚在介入手术中的应用,了解并证实其起效快苏醒快的优越性.方法 行介入手术病人30例,随机分为A、B两组.诱导时A组用瑞芬太尼1ug/kg,B组则用芬太尼5ug/kg.维持用药:A组病人用微量泵经静脉持续泵入瑞芬太尼(开始速度0.25ug/kg/min,逐渐减量至0.1ug/kg/min)和丙泊酚(开始速度8~10mg/kg/h,逐渐减量至5~7mg/kg/h),B组则按需间断追加芬太尼2~3ug/kg,并同A组一样经静脉持续泵入丙泊酚.观察记录苏醒时间、术后恢复评价指标即数字-符号替换测试评分DSST(digit-symbol substitution test score),并计算DSST恢复率:术后DSST评分/术前DSST评分×100%,记录麻醉药用量.结果 苏醒时间A组显著短于B组,且A组术后DSST评分显著高于B组.A组瑞芬太尼用量650±394ug,B组芬太尼用量458±157ug.A组丙泊酚用量150.4±33.9mg明显少于B组的173.7±46.4mg.结论 瑞芬太尼复合丙泊酚全凭静脉麻醉显著缩短了苏醒时间,显著提高了术后DSST恢复率,术后认知功能恢复较好,恢复清醒程度较高.因此特别适合切口小创伤小、术后疼痛较轻、无需术后镇痛的介入手术.
